Print and fill out the application form listed.

Options for processing the search are:

1. Scan and email the form to Community Development Services Commission
2. Drop off form in person to our offices located at 
   800 Research Drive
   Woodland Park, CO Suite Number 100
3. Mail form to:
   Teller County Community Development Services
   P.O. Box 1886
   Woodland Park, CO 80866

The fee of $10 will be collected prior to release of the information.

Accepted forms of payment are check, cash or credit card (credit card service fee is 99¢)
